I have tried to search for available contact form to be used on my blogger blog, unfortunately I have not found one even on using a widget. Few searches comes up on how to create one using different form creator website. But the catch is you need to register to them in order to create a form and forward an email if anyone fill the form up. Lazy on signing up, why not make use of google docs form feature to create one. Thats what I did.
Make sure you have a Google account, I am sure you have if you are using Gmail or other services provided by Google. Now go to http://docs.google.com or if you have a custom domain that is previously registered to them to be used on blogger you can use that too. Mine was set at http://docs.onemansgame.com.
Once you have logged in on google docs, create a new form as seen in the picture at the right. Select form, a new window opens and lets you modify the forms to suit your needs.
Repeat the process above if you want to add multiple info from the person accessing you contact form, you can add, email, subject line and just make use of Text as an option question type. Now create the large portion of the contact form, the body of the message. Click the Add Item above as seen by the image below using paragraph text.
Now you have almost complete simple form. See below. Save it.
Now click the More Options above and Click Embed to have the link to the form ready to be published on your blog. Grab that iframe code.
Now go to your blogger blog and create a new post. In the New Post, choose the Edit HTML tab and paste the embed code you've got above, published it and your are done. See example form at this blog. Click the Contact Me above or this link below, live in action.
